Progression of Neuropsychiatric Symptoms in Dementia with Lewy Bodies Lead Investigator: Lisa Wise Institution : UCSF Fresno E-Mail : lisa.wise@ucsf.edu Proposal ID : 1665 Proposal Description: Purpose: This study seeks to define the neuropsychiatric symptom categories in DLB and to determine how these are related to disease progression. This study is novel in that MCI due to DLB will be included for analysis and the relationship between apathy and depression in DLB will be explored. Design: Longitudinal analysis of neuropsychiatric profiles in participants with normal cognition at baseline, progressing through various stages of DLB including MCI due to DLB and DLB. Participants: NACC participants enrolled from 9/2005 - 12/2020, age 50 and above, with at least five follow up visits, diagnosed as having normal cognition at initial visit with eventual diagnosis of Dementia with Lewy Bodies. Exclusion criteria: History of major psychiatric disorder or major medical disorder thought to be the primary cause of NPS. Presence of comorbid dementia diagnosis other than DLB. Methods: Neuropsychiatric symptoms will be measured using informant rated NPI-Q, presence or absence of symptoms and severity rating, as well as clinician judgment. Depression will additionally be measured by Geriatric Depression Scale. For patients who have completed LBD Module, apathy will be further characterized using expanded NPI. For patients with current diagnosis of DLB, severity of impairment will be rated using the Clinical Dementia Rating(CDR) Scale. Analysis: We will factor analyze the data from the Neuropsychiatric Symptom Inventory (NPI) for each diagnostic group separately to determine the categories of symptoms that emerge. Then we will compare each group in their longitudinal changes in these factors. We will then examine changes in apathy and depression for individual subjects within each group to explore their correlation and progression over time.
